Do you believe in Whole Body Health? This is where one's health (including physical, mental, and emotional factors) enable a person's ability to thrive.

The Health Mentor is part of the InShape Program at West Central Behavioral Health, that combines physical fitness and dietary counseling designed specifically for people living with severe mental illness as a means to improve overall client health. The Health Mentor encourages involvement in activities, regular exercise, and better eating habits.

 

Responsibilities:

  • Manages a caseload of clients to establish and implement individual InSHAPE plans
  • Provides support and education related to physical fitness, nutrition and wellness.
  • Provides group activity instruction, such as: Yoga, Pilates, boxing,
  • Monitors and reassesses exercise, nutrition and health care commitments
  • Provide support, education and coaching around symptom management to assist clients with psychiatric symptoms that impair InSHAPE goals
  • Encourages member participation and movement along the continuum of wellness-related lifestyle changes
  • Administers fitness assessments as needed and records data
  • Collaborates with community partners who can provide services to clients, including gyms, grocery stores, nutritionists, as well as opportunities for health-related field trips.

The individual will provide services in both lower Grafton and all of Sullivan counties.

  • Bachelor’s degree in Health Science, Social Work, Rehabilitation, Psychology, Education or related field.
  • Current Personal Trainer Certification preferred, required within 90 days of hire
  • 1-2 years of experience working collaboratively with one or more individuals to help accomplish a goal or behavior change.
  • Excellent interpersonal/relationship building skills.
  • Excellent organizational skills: ability to manage own schedule with clients and group commitment, ability to manage a balance of direct care and documentation responsibilities.

Skills:

  • Ability to work independently without regular supervision.
  • Physically active, readily identifies and believes in the holistic benefits of good health.
  • Excellent computer skills with knowledge of Microsoft Office, especially PowerPoint.
  • Maintain skill competencies in customer service, quality improvement, compliance and safety.
  • Demonstrate effective ability in the supervision and direction of volunteers.
  • Ability to learn necessary computer skills essential for the working of electronic medical record, office software, and electronic communications.
  • Excellent communication skills.

 

Preferred Skills:

  • Video and web-based development experience

 

Physical Requirements:

  • Ability to lift, push and/or pull 15 lbs.
  • Ability to bend, stoop, work with hands and/or arms overhead for frequent periods.
  • Ability to access all Agency sites.

 

Other:

  • Have and maintain a valid State driver’s license.
  • Have reliable transportation.
  • Ability to obtain and maintain Agency and/or State of New Hampshire required background check(s)
  • Understanding of the importance of confidentiality and the ability to maintain that confidence
